 1                    AMENDMENT TO SENATE BILL 1664

 2        AMENDMENT NO.     .  Amend Senate Bill 1664  on  page  5,
 3    line  23,  immediately  after "activities.", by inserting the
 4    following:
 5    "Whenever records are disclosed pursuant to this  subdivision
 6    (12),  the  recipient  of  the  records  shall  be advised in
 7    writing that any person who discloses mental  health  records
 8    and communications in violation of this Act may be subject to
 9    civil  liability  pursuant  to  Section  15 of this Act or to
10    criminal penalties pursuant to Section  16  of  this  Act  or
11    both.".
