1 AN ACT concerning not-for-profit corporations.
2 Be it enacted by the People of the State of Illinois,
3 represented in the General Assembly:
4 Section 5. The General Not For Profit Corporation Act of
5 1986 is amended by adding Section 114.15 as follows:
6 (805 ILCS 105/114.15 new)
7 Sec. 114.15. Annual report; health care facility. Each
8 not-for-profit corporation operating a health care facility
9 in this State shall file with the Secretary of State a report
10 not later than July 1 of each year containing the following
11 information:
12 (1) The names, addresses, telephone numbers, and
13 occupations of all officers, members of the board of
14 directors, trustees, executive officers, and senior level
15 management personnel of the health care facility.
16 (2) The names, addresses, and telephone numbers of
17 professional consultants employed by the health care
18 facility.
19 (3) A copy of audited income statements, balance
20 sheets, and other financial statements for the
21 corporation's past fiscal year.
22 (4) The names and mailing addresses of all licensed
23 health care facilities in which the not-for-profit
24 corporation maintains an ownership interest or operating
25 authority.
26 (5) A description of charity care and uncompensated
27 services provided during the past fiscal year by each
28 health care facility, including an estimated value of
29 those services and how that value was calculated.
30 (6) A copy of the health care facility's Community
31 Benefits Plan developed in accordance with Section

1 501(c)(3) of the Internal Revenue Code and, in addition,
2 a list of services the health care facility plans to
3 provide in the next fiscal year to address community
4 health needs including: (a) free care; (b) services for
5 vulnerable populations; (c) health research, education,
6 and training; (d) services that address public health
7 needs; and (e) in-kind support and other services to the
8 community such as preferential hiring policies for
9 uninsured and underinsured individuals residing in the
10 community.
