| 3 | | with reference to page and line numbers of Senate Amendment No. |
| 4 | | 3, on page 2, lines 2 through 4, by replacing "with |
| 5 | | notification to the patient's physician and with appropriate |
| 6 | | reporting and record retention" with ". Vaccination of a |
| 7 | | patient by a dentist shall be documented in the patient's |
| 8 | | dental record and the record shall be retained in accordance |
| 9 | | with current dental recordkeeping standards. The dentist shall |
| 10 | | notify the patient's physician of each dose of vaccine |
| 11 | | administered to a patient and shall enter all patient level |
| 12 | | data or update the patient's current record in the immunization |
| 13 | | data registry maintained by the Department of Public Health. |
| 14 | | The dentist may provide this notice to the patient's physician |
| 15 | | electronically. The dentist shall also use the immunization |
| 16 | | data registry system to review vaccination history for a |
| 17 | | patient prior to the administration of any vaccine.".
