| 2 | | WHEREAS, The members of the Illinois House of |
| 3 | | Representatives saddened to learn of the death of Dr. John |
| 4 | | "Jack" Henry Boarini of Lake Villa, who passed away on May 27, |
| 5 | | 2014; and
|
| 6 | | WHEREAS, Dr. Boarini was born in Chicago on March 8, 1930 |
| 7 | | to Dr. Edward Vincent and Edelle (nee Odenbreck) Boarini; he |
| 8 | | married Nancy Grant on August 26, 1950; he graduated from the |
| 9 | | Loyola School of Dentistry in 1955 and entered the United |
| 10 | | States Air Force that same year; he was honorably discharged in |
| 11 | | 1958; and
|
| 12 | | WHEREAS, Dr. Boarini moved to Antioch 1968 and bought his |
| 13 | | dental practice in 1972 at Orchard Medical; he built the |
| 14 | | largest professional building in Antioch, The Heritage |
| 15 | | Building, in 1975; his dental office, Heritage Dental, is still |
| 16 | | serving the community; and
|
| 17 | | WHEREAS, Dr. Boarini was a member of the Lion's Club since |
| 18 | | the early 1970s; he helped initiate the eye wear collection |
| 19 | | boxes for the Lions in Antioch, which donated glasses and |
| 20 | | hearing aids to the needy; he was a Rotary International double |
| 21 | | Paul Harris Fellow and had been an active supporter of St. |
| 22 | | Peter Church and served as a School Board Member for 8 years; |

| 1 | | at his home on Maui, he founded a mobile dental unit for the |
| 2 | | poor and received the Father Damion Award from the State of |
| 3 | | Hawaii; after visiting St. Francis Catholic Mission in South |
| 4 | | Dakota on a hunting trip, he began work on opening a dental |
| 5 | | clinic there for Native Americans; and |
| 6 | | WHEREAS, Dr. Boarini was affectionately known by many |
| 7 | | monickers, including "the singing dentist", "the tireless |
| 8 | | volunteer", "the die-hard golfer", and "the never-ending giver |
| 9 | | to the needy"; he had a love for hunting, and, while in the Air |
| 10 | | Force in Wyoming, started the family tradition of annual big |
| 11 | | game hunts out west and was well known as a great hunter into |
| 12 | | his 83rd year; he was a fan of the Chicago Cubs and Chicago |
| 13 | | Bears; and
|
| 14 | | WHEREAS, Dr. Boarini was preceded in death by his parents; |
| 15 | | and
|
| 16 | | WHEREAS, Dr. Boarini and his wife were the parents of 8 |
| 17 | | children, Dr. David (Michelle), Dr. Mark (Kathy), Kathy (the |
| 18 | | late Ernie) Boggs, Chris Boarini, Paul (Mila), Karen Staten, |
| 19 | | Dr. Daniel J. (fiancée Kathryn), and Matthew (Barbara); 34 |
| 20 | | grandchildren; and 11 great-grandchildren; therefore, be it
|
| 21 | | RESOLVED, BY THE HOUSE OF REPRESENTATIVES OF THE |
| 22 | | NINETY-EIGHTH GENERAL ASSEMBLY OF THE STATE OF ILLINOIS, that |
