Defines "adult grandchild" as the biological grandchild 21 years of age or over of a deceased adopted or surrendered person. Replaces references to the Adoption Advisory Council with references to the Adoption Registry-Confidential Intermediary Advisory Council. Makes changes in Sections concerning the Illinois Adoption Registry and Medical Information Exchange; the Adoption Registry-Confidential Intermediary Advisory Council; disclosure of identifying information; registry matches; the Illinois Adoption Registry Application; and forms. In the Section concerning confidential intermediaries, makes changes in subsections concerning: general purposes; eligibility of petitioner; the petition; confidential intermediary access to information; remedies in the event of a missing or lost original birth certificate; and duties of the confidential intermediary in conducting a search.</SynopsisText><synopsistitle>House Floor Amendment No. 1</synopsistitle>
<SynopsisText>Adds birth parents of adult adopted persons to the list of parties who must comply with specified procedural requirements in making a request for a non-certified copy of the original birth certificate. Provides that beginning on January 1, 2015, any birth parent of an adult adopted person named on the original birth certificate may request a non-certified copy of the original birth certificate reflecting the birth of the adult adopted person, provided that: (1) any non-certified copy of the original birth certificate released under this subsection (m) shall not reflect the State file number on the original birth certificate; and (2) if the Department of Public Health does not locate the original birth certificate, it shall issue a certification of no record found. Deletes language providing that confidential intermediary fees are waived only if specified conditions are met. Provides instead that no fee shall be charged to any petitioner.</SynopsisText></synopsis>
