| AMENDMENT TO HOUSE BILL 5382

| AMENDMENT NO. ______. Amend House Bill 5382 on page 1, by |

| replacing lines 21 and 22 with the following: |

| "setting to another type of residential setting without his or |
| 5 |
| her voluntary informed consent and the voluntary informed |
| 6 |
| consent of his or her guardian (if one exists)."; and
|
| 7 |
| on page 13, line 28, after the period, by inserting "After the |
| 8 |
| submission of each annual report, a public hearing before the |
| 9 |
| appropriate committee of the Illinois General Assembly shall be |
| 10 |
| held to receive public comment and further discussion of the |
| 11 |
| Disability Services Plan.".
