AMENDMENT TO HOUSE BILL 2453

    AMENDMENT NO. ______. Amend House Bill 2453 on page 1, by

replacing lines 9 and 10 with the following:

"post-hospital care, whenever a patient who qualifies for the 
5
federal Medicare program is hospitalized, the patient shall 
6
be"; and

on page 1, line 12, by deleting "skilled"; and
 
8
on page 1, by replacing lines 14 and 15 with the following:

"hospital must inform the designated case coordination unit or 
10
other pre-screening entity of the pending discharge"; and
 
11
on page 1, line 16, after "unit's", by inserting "or other 
12
pre-screening entity's"; and
 
13
on page 1, line 26, by changing "assisted living and shared 
14
housing" to "assisted living or shared housing"; and
 
15
on page 1, line 30, by deleting "or"; and
 
16
on page 1, line 31, after "Act", by inserting "; or (vi) any 
17
agency licensed under the Home Health Agency Licensing Act";
