Requires evidences of coverage to contain a clear and complete statement of deductibles. Provides that HMOs may establish annual deductibles not to exceed certain amounts. Provides that co-payments may not exceed 50% of the usual and customary fee charged to the HMO for the service and provides that deductibles are not subject to this limitation. Makes other changes.
