Provides that an applicant for benefits under Medicaid or the Children's Health Insurance Program, or a person requesting uncompensated care in a hospital, must identify the employer or employers of the proposed beneficiary of the health care benefits provided under that program. Requires the Department of Public Aid to submit to the General Assembly an annual report, beginning in 2007, identifying all such identified employers who employ 25 or more public health program beneficiaries. <SynopsisText>Replaces the provisions concerning the reporting of employer-provided health insurance information. Provides that the Department of Public Aid or its successor agency, in collaboration with the Department of Human Services and the Department of Financial and Professional Regulation (instead of only the Department of Public Aid), shall annually prepare a public health access program beneficiary employer report to be submitted to the General Assembly. Requires the reporting of information for each employer who has more than 100 employees and 50 or more public health access beneficiaries (instead of 25 or more public health program beneficiaries). Specifies the information to be included in the report, adding information concerning health insurance benefits offered by the employer.
