| AMENDMENT TO HOUSE BILL 2633

| AMENDMENT NO. ______. Amend House Bill 2633 on page 1, by |

| replacing lines 12 through 14 with the following: |

| "year after the date of payment or one year after the funds are |
| 5 |
| last used for any unauthorized purpose, whichever is later. |
| 6 |
| Each request must state: (1) the name and address"; and

| on page 2, by replacing lines 3 through 5 with the following: |

| "within one year after the date of payment or one year after |
| 9 |
| the funds are last used for any unauthorized purpose, whichever |
| 10 |
| is later. Each request must state: (1) the name and address".
