AMENDMENT TO SENATE BILL 2215 

2    AMENDMENT NO. ______. Amend Senate Bill 2215 by replacing 
3everything after the enacting clause with the following:
 
4    "Section 5. The Department of Public Health Act is amended 
5by adding Section 8.5 as follows:
 
6    (20 ILCS 2305/8.5 new)
7    Sec. 8.5. Department to provide information for health 
8care professionals to distribute to women over 25 concerning 
9fertility options.
10    (a) As used in this Section, "health care professional" 
11means a physician licensed under the Medical Practice Act of 
121987, a podiatrist licensed under the Podiatric Medical 
13Practice Act of 1987, an advanced practice registered nurse 
14licensed under the Nurse Practice Act, or a physician 
15assistant licensed under the Physician Assistant Practice Act 
16of 1987.

1    (b) The Department of Public Health shall provide the 
2following information for health care professionals to 
3distribute to women over the age of 25 years, or women 
4interested about fertility options on blood tests to predict 
5their ovarian reserve:
6        (1) a description of the concept of ovarian reserve;
7        (2) an overview of different types of ovarian reserve 
8    testing;
9        (3) a list of healthcare centers or hospitals that are 
10    available to women for ovarian reserve testing; and 
11        (4) information on potential results and resources 
12    that are available after testing.".
