AMENDMENT TO SENATE BILL 1076 

2    AMENDMENT NO. ______. Amend Senate Bill 1076 by replacing 
3everything after the enacting clause with the following:
 
4    "Section 5. The Department of Insurance Law of the Civil 
5Administrative Code of Illinois is amended by adding Section 
61405-16 as follows:
 
7    (20 ILCS 1405/1405-16 new)
8    Sec. 1405-16. Medicare consumer checklist.
9    (a) The Department of Insurance shall develop and 
10administer a one-page consumer checklist comparing Medicare 
11Advantage plans and Medicare Supplement plans. The checklist 
12shall compare:
13        (1) plan networks, including differences in providers, 
14    specialists, hospitals, urgent care centers, and 
15    pharmacies;
16        (2) portability rules, including rules for travel,

1    out-of-state coverage, and temporary out-of-area coverage;
2        (3) prescription drug coverage, including drug 
3    formularies, cost-sharing, mail-order pharmacy coverage, 
4    prior authorization requirements, and step therapy;
5        (4) maximum out-of-pocket exposure, including annual 
6    limits, in-network and out-of-network expenses, and 
7    cost-sharing for services; and
8        (5) additional benefits and services.
9    (b) The Department shall update the checklist under this 
10Section on January 1 of each year, beginning on January 1 of 
11the year after the effective date of this amendatory Act of the 
12104th General Assembly.".
