1 AMENDMENT TO HOUSE BILL 4434

2 AMENDMENT NO. ______. Amend House Bill 4434 on page 3, by

3 replacing lines lines 13 and 14 with the following:

4 "individuals."; and

5 on page 6, by replacing line 11 with the following:

6 "(B) Recognizing the uniqueness of each facility,

7 services shall be available in each facility"; and

8 on page 6, line 12, by replacing "house" with "houses"; and

9 on page 8, line 4, by deleting "legal documents;"; and

10 on page 12, line 5, by inserting after period the following:

11 "Health care professionals qualified to complete a mental

12 health evaluation and depression screening include: (i)

13 psychiatrists; (ii) clinical psychologists; (iii) clinical

14 social workers; or (iv) advanced practice registered nurses.

1 The mental health evaluation and depression screening must be

2 completed within 30 days of being requested, or the individual

3 must be allowed to seek the mental health evaluation and

4 depression screening from an outside provider if the facility

5 does not have a provider within 30 days of being requested.";

6 and

7 on page 13, by replacing lines 9 through 12 with the following:

8 "(1) the Individual Benefit Fund; and

9 (2) federal appropriations if applicable.".
