| 3 | | line 20, by deleting "the policy is in full alignment with |

| 5 | | on page 5, line 7, after the semicolon, by inserting "the |
| 6 | | ordering provider is not required to obtain continuing medical |
| 7 | | education in order to prescribe a continuous glucose |
| 8 | | monitor;"; and
|
| 9 | | on page 6, by replacing lines 3 through 6 with the following: |
| 10 | | "(7) prior authorization is required for a |
| 11 | | prescription of a continuous glucose monitor; once a |
| 12 | | continuous glucose monitor is prescribed, the prior |
| 13 | | authorization shall be approved for a 12-month period.". |
