Prohibits the Department of Healthcare and Family Services from expanding care coordination to recipients of medical assistance who: (i) receive care at facilities licensed under the Nursing Home Care Act, the MC/DD Act, or the ID/DD Community Care Act, or at facilities authorized as supportive-living facilities under a specified provision of the Code; and (ii) are not enrolled in the Medicare-Medicaid Alignment Initiative Program.
